SURVEYS

Dear Participant:

The following Informed Consent Form explains what an NSDL survey is, how it works, and how your privacy will be protected. We want you to understand what you are being asked to do and what risks and benefits - if any - are associated with the survey. This will help you decide whether or not you want to participate in the survey.

In the case of written surveys, a written consent form is signed and returned by the survey participant. In an online survey, it will not be possible to obtain your signature. Instead, your decision to participate in and to complete the survey will be taken as evidence that you have read the following document, and that have been informed about the survey and that you want to participate.

YOUR PARTICIPATION IN THIS SURVEY IS VOLUNTARY. PLEASE READ THE DESCRIPTION OF THE SURVEY THAT FOLLOWS. IF YOU DO NOT WISH TO PARTICIPATE, PLEASE DO NOT START THE SURVEY
